Accident Investigation Procedure

  • Internal
  • • All drivers with accidents are suspended with immediate effect. (KBSM Ltd)
  • • They report to the accident investigators the following day for statement recording.
  • • The accident is investigated accordingly and recorded in the Accident claim form provided.
  • • If injury or fatal, the crew may be required to accompany the investigator to the site.
  • • They also go to the external investigator of appointed for further interviews.
  • • All documents given to the driver at the Police Station must be given to the Investigator.
  • • The driver is required to have reported all accidents to the Police unless Third Party admits liability in writing if the accident is non injury.
  • • If third party is at fault for a collision accident the driver is issued with a blank PAR by the Investigator to be returned duly executed by Police in the Station where the accident was reported.
  • • Once the investigations are complete, the information is entered in a summary register, Cost of Repair (COR.) attached and then passed to Claims Office via another register.
  • External Investigator
  • Appointed for all multiple injuries, fatalities and any accident deemed serious plus alleged accidents.
  • Instruction through the claims office or radio controller in case of non working hours.
  • Crew sent to investigator for statement.
  • Internal investigator must be engaged as well.
  • Internal report within 24 hours.
  • Final report within 7 days almost 2 weeks.
  • Fees pre-agreed.
  • No deposits to attend Court.
  • Investigators provide their own transport.
  • Police Abstracts
  • Buy our own booklets from Police Station.
  • Issued for Third party particulars.
  • Issued to determine fault where our driver is suspected not to be at fault.
  • Issued to crew for completion by Police.
  • To be returned within 30 days after issue whether blank or completed.
  • 200/ = recovered from the defaulters.
  • Once received, forward to claims office for action.
  • Cost of Repair to Bus
  • Request daily from  workshop through a routine form
  • To be received within 3 days.
  • All files to claim Office must be accompanied by Cost of Repair unless not applicable.
  • Follow up done through the body superintendent.
  • If there is a daily physical follow up is done and also reminder list is passed to the concerned engineer.
  • Workshop Report
  • Mainly on alleged defects i.e brakes, steering etc.
  • Requested upon allegations through a routine designed form.
  • Must be returned within 48 hours after request or else it become irrelevant.
  • Used to determined fault where Mechanical defect is alleged.
  • Forwarded together with the claim forms to claims Office for action.
  • If vehicle found defective, corrective action ought to be indicated.
  • Accident Summary
  • Periodic report.
  • Done to Traffic Manager showing verdicts and the crew involved.
  • Used for disciplinary action against the drivers.
  • Include details of the crew particulars nature of accident and who was to blame.
  • Court Attendance
  • By court clerks.
  • For all Company/Management charges.
  • Also for crew especially if the Company is defending.
  • A court diary is maintained to monitor the hearings.
  • Two clerks deal.
  • Claims Office facilities payment of cash bail to bail the court clerks and or drivers out.
  • The court clerks also process crew allowances to attend court via staff advice.
  • Charges :
  • All at fault drivers are charged by the investigating Officer.
  • The charge sheet is then passed over to claims office for the disciplinary case to be carried out.
  • It shows the nature of offence committed.
  • The Disciplinary Officer will then uplift the suspension after he is done with the case.
